Promoting Medicare Annual Wellness Visits, 2025-2027
Overview
The Medicare Annual Wellness Visit (AWV) is a preventive care appointment. The essential purpose is to prevent the onset of disease and disability by slowing the progression of existing or future illnesses. This module explains the benefits and requirements of the Medicare Annual Wellness Visit.
Medicare covers many preventive services. Understanding what services are available, the person receiving care and their healthcare provider can create a schedule for future appointments. Clinics can also increase revenue by recommending further tests, screenings, etc., and attaining high-quality care metrics.
Target Audience
This activity is intended for physicians, nurses, pharmacists, social workers, and physical therapists.
Learning Objectives
Upon completion of this activity, learners should be able to:
- Differentiate between the Medicare Annual Wellness Visit and the concept of an "annual physical".
- List the benefits of the Medicare Annual Wellness Visit to the patient and manage patient expectations for the visit.
- Describe the benefits of maintaining a high participation percentage within the Medicare beneficiary population.
- Identify common documentation errors that can result in payment denials from Medicare.
- Discuss how aligning care with the IHI Age-Friendly Health System and an interprofessional approach helps eliminate health disparities for older adults.
