Jennifer Young, MSN, BSN, RN, ACM-RN

Jennifer Young is a lifelong North Texan, raised in West Fort Worth and now residing in Weatherford, Texas. She earned her Bachelor of Science in Nursing from Texas Christian University in 2005 and went on to complete her Master of Science in Nursing with a focus on Management and Leadership from Walden University in 2013.
Jennifer began her nursing career in Labor and Delivery before transitioning to the Neonatal Intensive Care Unit (NICU). As she and her husband, Jake—both working in shift-based service professions—started their family, Jennifer sought a more balanced path. This led her to Case Management in 2010, beginning with roles in Texas Medicaid programs, then moving into Long-Term Acute Care, and ultimately joining Texas Health Resources in the Care Transitions Department in 2016.
Currently serving as the System Director of Care Transitions Management at Texas Health Resources, Jennifer is known for her innovative and evidence-based approach to improving care delivery. She is deeply committed to enhancing patient outcomes through strategic process improvements in length of stay management and readmission reduction, ensuring high-quality, efficient care for patients, staff, and physicians alike.
Jennifer is also an active leader in professional organizations. She has volunteered extensively with the American Case Management Association (ACMA), where she has served as Membership Chair, Conference Planning Chair, and most recently, as President for three years. She was recently appointed to the Women Healthcare Executives Networking (WHEN) Committee of the American College of Healthcare Executives (ACHE).
Outside of her professional life, Jennifer and Jake recently celebrated their 19th wedding anniversary on February 4, 2025. They are proud parents to three energetic boys—Fisher (16), Chord (13), and Judge (8). When she’s not leading hospital initiatives or supporting her community, Jennifer can be found cheering from the sidelines at the many (many!) sporting events her sons participate in.
