HSC Center for Older Adults: 2023-25 Supporting Persons Living with Dementia and Their Caregivers

This online learning module is intended for a multispecialty audience. Early recognition of dementia can help reduce the emotional, physical, and financial impact on those with dementia and their caregivers. Healthcare providers across the continuum of care may pick up on clues that a further discussion or referral is needed.

This module will provide basic information on Alzheimer's Disease and related dementias. Additionally, this module offers guidance on using the Chronic and Principle Care Management billing codes to support care coordination for patients with dementia and their caregivers. The Alzheimer's Association provides many resources for clinicians, patients, and caregivers for the breadth of dementia diagnosis, not just Alzheimer's Disease.

Target Audience

This activity is intended for physicians, nurses, pharmacists, pharmacy technicians, certified public health professionals, social workers, physical therapists, licensed professional counselors, licensed marriage and family therapists and psychologists.

Learning Objectives

When you complete this module, you will be able to:

  • Recognize the benefits of identifying dementias early.
  • Describe key differences between dementias and normal aging.
  • Evaluate resources for adjunctive interventions that integrate the interprofessional healthcare team to support those living with dementia and their caregivers.
  • Review Medicare billing options (Dementia Care Plan Code, CCM, PCM) and analyze how these programs can improve the healthcare team's ability to provide Age-Friendly Health Care and improve care access.
