High-Risk Medication in the Elderly (Center for Older Adults 2026-2028)

This online learning module will address the characteristics of medications that are high-risk and how to mitigate those risks for older adult patients. 

Target Audience

This activity is intended for physicians, physician associates/physician assistants, nurse practitioners, nurses, and pharmacists 

Learning Objectives

When you complete this module, you will be able to:

  • List medication groups considered high risk for the elderly.
  • Review the characteristics of a high-risk medication.
  • Apply an age-friendly framework to patient-centered prescribing and deprescribing.
  • Differentiate medication reconciliation from medication review and describe how prescribers, pharmacists, nurses, and medical assistants collaborate to ensure accurate and safe medication management.
