SPH: 2024-2026 CHW Health Screening and Coordination Skills
Health Screening & Coordination Skills are abilities CHWs use when assessing, screening, and caring for patients. These skills also describe applied medical knowledge, such as assessing blood pressure, cholesterol or blood glucose results. CHWs can develop these skills using this module and then, gain hands-on experience working with clients.
Health Screening & Coordination Skills can be hard or soft skills. Soft ones, such as communication, are important for interacting with patients and gathering enough information to screen and refer them accurately. Hard skills are more technical abilities that are specific to the CHW role, such as taking or reading vital signs or conducting screenings.
Target Audience
This activity is intended for preparing community health workers.
Learning Objectives
When you complete this module, you will be able to:
- Demonstrate knowledge of measuring, monitoring, and explaining the basic clinical vital signs.
- Describe the impact of clinical indicators and measures on long-term health.
- Employ effective strategies to communicate and counsel clients on these impacts and potential lifestyle modifications.
- Identify resources that support and enhance the education on health and well-being for clients.
- Coordinate screening, measures, and outcomes with clients’ providers to facilitate patient-centered and coordinated care.
