HSC Health: 2022-24 Value-Based Care Quality Measures Update

Value-based care is the concept that healthcare professionals should be reimbursed based on the quality of care they provide instead of the volume of services. The goal is to support individuals at the highest possible level of wellness rather than wait until individuals are sick, which is often more complex and expensive. Evidence-based preventive care, such as cancer screenings, chronic disease management programs, and regular wellness assessments, have been proven to catch rising risks early enough to avoid hospitalizations, emergency department visits, and other high-cost interventions.

Target Audience

This activity is intended for physicians, nurses, pharmacists, certified public health professionals, social workers, physical therapists, licensed professional counselors, licensed marriage and family therapists and psychologists.
