HSC Health: 2021-24 Population Health - Caring Beyond our Walls

Published literature demonstrates the importance of understanding social determinants of health (SDH) and the need for healthcare providers to address underlying SDH for successful patient outcomes. This course will focus on reasons for taking a population health approach in clinical practice and how recognizing and addressing social determinants of health within the patient encounter can improve health outcomes in daily practice.

Target Audience

This activity is intended for physicians, nurses, pharmacists, certified public health professionals, social workers, physical therapists, licensed professional counselors, licensed marriage and family therapists and psychologists.
