Goldstein Lecture (ORC): Use of LLM and AI in Pain Research: Opportunities and Obstacles
It may be possible to use generative AI and Large Language Models (LLMs) across multiple domains of chronic pain research including mining electronic health records for pain phenotypes and treatment outcomes, extracting structured information from patient-reported symptoms in natural language, automating literature synthesis to identify knowledge gaps, and developing and testing patient-facing tools such as educational chatbots and symptom diaries. The goal is to use generative AI and LLMs to conduct both qualitative and quantitative research. There is potential to use these tools to analyze patient written narratives describing their pain and various self-management and self-care strategies employed by the patients. The tools can integrate genomic, imaging, and clinical data to generate and test these hypotheses. The most significant obstacle is that experts have not evaluated the quality of data used to train these models. Training data biases may cause pain conditions and demographics to be underrepresented. There are HIPPA and GDPR restrictions while handling protected patient data. We will discuss how these tools can be used for literature review and de-identified data exploration. We will also discuss available tools that utilize local or on-premise models for sensitive data. We have published extensively from the PRECISION registry and we want to explore the possibility of taking a second look at the same questions and data using LLMs.
