James L. West Center for Dementia Care: 2025-2027 Building Care Together: Collaborative Assessments & Planning

This interdisciplinary course is designed for direct care and supportive healthcare professionals across diverse care settings and equips them with the skills and knowledge to deliver collaborative, person-centered dementia care. Learners will explore the complexity of dementia, the importance of interdisciplinary teamwork, and the use of holistic assessments to guide co-created care planning. The course also covers effective communication strategies, safety and advocacy protocols, and use of technology such as remote monitoring, wearable devices, and surveillance tools. By the end of the course, participants will be prepared to support coordinated, individualized care that enhances quality of life for individuals living with dementia and strengthens team-based practices across care environments.

Target Audience

This activity is intended for: physician assistants, nurses, social workers, community health workers, public health professionals, Texas-licensed physical therapists, and Texas-licensed professional counselor.

Learning Objectives

By the end of this course, you should be able to: 

1. Describe how interdisciplinary, person-centered care addresses the growing complexity of dementia across care settings.
2. Identify appropriate cognitive, functional, emotional, and environmental supports and assessments used in dementia care planning.
3. Demonstrate communication strategies that promote effective collaboration across healthcare roles and settings
