HSC Center for Older Adults: 2022-24 Home-based Primary Care

An estimated 1.9 million U.S. adults age 65 or older are completely or mostly homebound. Home-based primary care (HBPC) provided by multidisciplinary teams to vulnerable, homebound older adults has been shown to reduce costs, with high rates of patient satisfaction and reduced symptom burden.

At the completion of this activity, the participant will be able to: 1) Identify patients who are candidates for home-based primary care and describe how to perform a house call. 2) Describe how to perform a home safety assessment. 3) Identify who is eligible for home health services. 4) Define the Medicare regulations regarding home health services. 5) Discuss how MIPS measures align with Age-Friendly Care and support quality care delivered by the interprofessional team.
