CHW: Older Adult Care - Mentation Focus 2024-2026

Program Overview

This activity will prepare community health workers to connect to people in the community and understand their health care needs. Ways to help community members to access the available health resources will also be covered. Many deaths and events of serious harm happening in U.S. healthcare are preventable. These poor health outcomes happen for several reasons. These reasons include the Social Determinants of Health (SDOH). SDOH can be a lack of access to healthy food, limited access to health care, and low income. They can also include culture, language, education, and low health literacy. Community Health Workers (CHWs) create good health and health behaviors by helping people access health resources. This course prepares you to connect people in your community with easy to understand health information.

Learning Goal and Outcome
Learning Goal: Participants will increase knowledge about the social determinants of health and how it impacts the health outcomes in the community.

Learning Outcome: Participants will connect the community members to the available health resources to improve their health outcomes.

Target Audience

This activity is intended for community health workers.

Learning Objectives

When you complete this module, you will be able to:

  1. Understand a variety of hard and soft interactive education methods for different learning styles and ages.
    • (Knowledge Based - 1.5 hour)

  2. Deploy education methods to improve knowledge and/or change behavior.

    • (Communication - 1.0 hour)

  3. Understand both hard and soft interactive education methods, tailored for different learning styles and age groups, to effectively communicate dementia-related information in a health-literate manner.
