Revamp Your Approach: Connecting to Resources

This FREE continuing education course is designed to enhance your ability to support patients with pain management and substance use disorders (SUD) by learning how to leverage community resources effectively. You will receive practical guidance identifying key care team partners within your community to expand treatment options and improve patient outcomes. You’ll explore how social determinants of health (SDOH) impact access to resources and lead to diverse health outcomes, gaining insight into structural barriers that patients may face.

Also, you will develop personalized strategies for identifying local resources and building essential partnerships to facilitate patient access and referrals. Through facilitated group discussions, you’ll reflect on your own practices, uncover areas for growth, and synthesize new approaches for integrating community resources into your care setting. Join us to strengthen your community connections and adopt a more collaborative, person-centered approach to pain management.

Target Audience

This course is intended for: Physicians, Nurses, Pharmacists, Social Workers, Public Health Professionals, Physical Therapists, Physician Assistants, Nurse Practitioners, Licensed Professional Counselors, Licensed Marriage and Family Therapists. 

Learning Objectives

At the conclusion of this activity, participants should be able to:

  1. Identify care team partners within your community to expand care opportunities for pain management and SUD. 
  2. Explain how structural barriers in accessing resources vary based on SDOH and the differing health outcomes for patients. 
  3. Describe plans to identify resources within your practice location and develop necessary relationships for patient access/referral. 
  4. Analyze personal CE participation in facilitated group discussion to identify personal practice gaps in pain management and SUD attitudes and behaviors. 
  5. Synthesize discussed resources to adopt an approach to their personal practice setting.
