Overview

By 2050, adults over the age of 65 will make up 20 percent of the U.S. population. This rapidly increasing segment of the population creates challenges for the healthcare workforce as older adults have unique clinical, behavioral, and psychosocial needs. JPS Geriatric Medicine and Hospice and Palliative Care programs will focus on common and emerging conditions that affect this specific patient population. It will promote evidence-based practice for older adults in all care settings at JPS Health Network, using the Age Friendly Health systems framework when applicable. As leaders in this subject matter, our goal is to educating our clinician workforce to reduce variability and follow evidence based practice, improve communication and optimize interprofessional teamwork.

Series Learning Objectives

  • Apply 4M framework for age-friendly care;
  • Describe geriatric evidence-based practices to benefit older adults at JPS;
  • Identify strategies to improve care for vulnerable populations; and
  • Explain the need for early ID and assessments of patients with life threatening illnesses who would benefit for early palliative care
