Series Learning Objectives

At the conclusion of this activity, participants should be able to:

  • Apply the 4 M framework for age friendly healthcare;
  • Describe geriatric evidence-based practices to benefit older adults at JPS;
  • Identify strategies to improve care for vulnerable patient population; and
  • Explain the need for early identification and assessment of patients with life-threatening illnesses who would benefit for early palliative care

 

In support of improving patient care, this activity has been planned and implemented by The University of North Texas Health Science Center at Fort Worth and JPS Health Network. The University of North Texas Health Science Center at Fort Worth is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing Education for the healthcare team. 

As a Jointly Accredited Organization, The University of North Texas Health Science Center at Fort Worth is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program.
