At the conclusion of this activity, participants should be able to:

• Apply the 4 M framework for age friendly healthcare;
• Describe geriatric evidence-based practices to benefit older adults at JPS;
• Identify strategies to improve care for vulnerable patient population; and
• Explain the need for early identification and assessment of patients with life-threatening illnesses who would benefit for early palliative care.
