NTACHC-Breaking Down Barriers: Strategies for Overcoming Systemic Healthcare Inequities in Integration Efforts

This mini conference is to focus on the interface between Health/Behavioral Health Integration via the Collaborative Care Model, SDOH & Equity. Holistic, quality care which is at the core of NTACHC’s mission. As Tarrant County's only Federally Qualified Health Center (FQHC) serving quite a diverse community, this conference will highlight the challenges, related solutions and opportunities moving forward in today’s rapidly evolving healthcare environment.  We are bringing together professionals from across the healthcare spectrum to discuss, explore and advance the use of integrated behavioral health systems and practices to improve patient care and outcomes.

Target Audience

This activity is intended for physicians, nurses, pharmacists, certified public health professionals, social workers, and licensed professional counselors.

Learning Objectives

Upon completion of this activity leaners should be able to:

 

  • Explain how integrated healthcare emphasizes patient centered care, shared decision making and patient engagement into treatment process.
  • Describe how integrated healthcare can address health disparities in access, and promote healthcare equity,
  • Describe how digital health solutions, EMRs,  telemedicine and other technological advancements facilitate integrated healthcare delivery.
  • Integrate evidenced based practices and innovative approaches to improve patient experiences and optimize resource utilization.
