Jacob Rogers, Pharm.D., BCACP
I earned a Bachelor of Science degree in Biomedical Sciences from Texas A&M University with a minor in business administration. I then completed my Doctor of Pharmacy degree at Texas Tech University Health Sciences Center Jerry H. Hodge School of Pharmacy in Abilene, Texas. I went on to complete a PGY1 residency at the Columbia VA Health Care System in Columbia, South Carolina and a PGY2 Ambulatory Care residency at JPS Health Network in Fort Worth, Texas. I also completed a Clinical Teaching Certificate program at the University of South Carolina School of Pharmacy. My PGY2 residency training focused primarily on managing diabetes mellitus, hypertension, dyslipidemia, anticoagulation, and heart failure in a diverse underserved population. I joined the HSC College of Pharmacy upon completion of my PGY2 training, and I am a Board Certified Ambulatory Care Pharmacist (BCACP). I currently practice as a clinical pharmacist in the Center for Older Adults at the HSC Health Clinic. In this role I provide pharmacotherapy management services under a collaborative drug therapy management protocol. I am a member of American College of Clinical Pharmacy (ACCP), American Association of Colleges of Pharmacy (AACP), American Society of Health-System Pharmacists (ASHP), and Phi Lambda Sigma Leadership Society (PLS).
