Target Audience

This activity was planned to address the educational needs of physicians, physician assistants, nurse practitioners, nurses, pharmacists, social workers, licensed professional therapists, public health professionals, physical therapists and other health care professionals.

  • Identify the roles and relationships between population health and Health Care Delivery Systems.
  • Describe early outcomes of a suite of community-based population health interventions. 
  • Select ways to better incorporate preventative health tools and evaluate their worth.
