Needs Statement

The patient centered medical home (PCMH) model provides an opportunity for the provider to collaborate with the patient and the other members of the care team.  In order to successfully move to the patient-centered care model, the foundation of the practice needs to be strong.  This module will take a provider through the steps of developing a strong foundation.

Target Audience

This elearning activity is intended for medical professionals (physicians, pharmacists, NPs, PAs, nurses) interested in the Primary Care Medical Home model.  

Objectives

By the completion of this activity, learners will be able to:

  • compare team models;
  • select a model best suited for their practice;
  • define team roles; and
  • assign responsibilities to team members.
